Your First Appointment at Family Dentistry

For your first appointment with Family Dentistry, please arrive 15 minutes before your scheduled time, bring your insurance card and a photo ID. For your convenience, you can click, download, and print any necessary forms and complete them at home instead of filling them out in our office. If you can't complete them at home, please arrive 30 minutes before your scheduled time.
